Sevindirici haber ise, uzmanlar\u0131n sonunda kronik a\u011fr\u0131 mekanizmas\u0131n\u0131 anlamalar\u0131 ve yeni tedavilerle -hem ila\u00e7, hem aletlerle- opioidlere alternatif g\u00fc\u00e7l\u00fc bir tedavi protokol\u00fc olu\u015fturmalar\u0131d\u0131r.<\/p>\n<p><strong>Opioidlerin etki\/etkisizle\u015fme mekanizmas\u0131 ve yeni opioidler<\/strong><\/p>\n<p>Opioidler etkilerini, omurilik ve beyinde yer alan a\u011fr\u0131 n\u00f6ronlar\u0131n\u0131n \u00b5 al\u0131c\u0131lar\u0131n\u0131 (opioidlerin a\u011fr\u0131 kesici etkilerinin b\u00fcy\u00fck b\u00f6l\u00fcm\u00fcnden sorumlu olan, ama ayn\u0131 zamanda ba\u011f\u0131ml\u0131l\u0131k riskinden de sorumlu al\u0131c\u0131lar=resept\u00f6rler) hedefleyerek g\u00f6sterir. Bu al\u0131c\u0131lara ba\u011flanan opioid, uyar\u0131y\u0131 etkisiz hale getirecek \u015fekilde n\u00f6ronlar aras\u0131 n\u00f6rotransmitter (sinir h\u00fccreleri aras\u0131nda ileti\u015fimi sa\u011flayan beyin kimyasal\u0131) sal\u0131n\u0131m\u0131n\u0131 durdurur. Russo\u2019ya g\u00f6re g\u00fc\u00e7l\u00fc opioidler, ge\u00e7ici a\u011fr\u0131 tedavisi ve akut a\u011fr\u0131da harika etki yarat\u0131r.<\/p>\n<p>Fakat kronik a\u011fr\u0131n\u0131n uzun s\u00fcreli tedavisinde yan etkiler durumu daha da k\u00f6t\u00fcle\u015ftirir. V\u00fccut kolayca tolerans geli\u015ftirir (ilaca tepkisiz kal\u0131r) ve doktorlar da bu durumda ilac\u0131n dozunu artt\u0131rmay\u0131 istemez. Nitekim Russo, Michael\u2019i hasta olarak g\u00f6rd\u00fc\u011f\u00fcnde ilk yapt\u0131\u011f\u0131 \u015fey opioid dozlar\u0131n\u0131 azaltmak olmu\u015ftu.<\/p>\n<p>Opioidlere kar\u015f\u0131 tolerans geli\u015fimi mekanizmas\u0131n\u0131 anlamak, ara\u015ft\u0131rmac\u0131lar\u0131, onlar\u0131 kullanmaktan ka\u00e7\u0131nmaya y\u00f6nlendirecektir. \u0130lac\u0131n \u00e7o\u011fu v\u00fccuda girdi\u011finde ilk olarak non-n\u00f6ronal h\u00fccreler olarak bilinen glia h\u00fccreleri taraf\u0131ndan fark edilir. Daha \u00f6nceleri sadece n\u00f6ronlar i\u00e7in \u00e7at\u0131 g\u00f6revi g\u00f6rd\u00fc\u011f\u00fc d\u00fc\u015f\u00fcn\u00fclen glia h\u00fccrelerinin art\u0131k merkezi sinir sisteminin aktif bir \u00fcyesi oldu\u011fu bilinmektedir. Bu h\u00fccrelerin g\u00f6revlerinden biri, v\u00fccuda giren yabanc\u0131 ziyaret\u00e7ileri izlemektir. Y\u00fcksek dozda opioid v\u00fccuda al\u0131nd\u0131\u011f\u0131nda, glia h\u00fccrelerinden imm\u00fcn (ba\u011f\u0131\u015f\u0131kl\u0131k) i\u015faret materyali olan \u201cinflamatuvarsitokin\u201d salg\u0131latarak, bir \u00e7e\u015fit savunma yan\u0131t\u0131 olu\u015fturur. Bu yan\u0131t\u0131n da duyusal n\u00f6ronlar\u0131 uyararak ilac\u0131n \u201cyat\u0131\u015ft\u0131r\u0131c\u0131\u201d etkisini g\u00f6stermesi beklenecektir. Ara\u015ft\u0131rmac\u0131lar, \u00b5 resept\u00f6rlerini zay\u0131f\u00e7a aktive edecek, fakat ayn\u0131 zamanda di\u011fer hedefleri de tetikleyebilecek ila\u00e7lar aray\u0131\u015f\u0131ndalar. \u00b5 resept\u00f6rlerini aktifle\u015ftirmek, a\u011fr\u0131 uyar\u0131s\u0131n\u0131n beyindeki ak\u0131\u015f\u0131n\u0131 bozacakt\u0131r. Opium bitkisi yerine i\u015fe beyinden ba\u015flamay\u0131 tercih etti. \u0130lk b\u00fcy\u00fck sonucu 1997\u2019de elde ettiler. Zadina ve ekibi, beyinde morfine oldu\u011fu kadar se\u00e7ici olabilen ve \u00b5 al\u0131c\u0131lar\u0131na ba\u011flanan bir peptidin (bir \u00e7e\u015fit protein) izini buldular. Bu bile\u015fim endomorfin-1 ad\u0131n\u0131 ald\u0131 ve \u015fimdilerde \u00b5 al\u0131c\u0131lar\u0131n\u0131n do\u011fal tetikleyicisi olarak biliniyor.<\/p>\n<p>Endomorfin-1, morfin kadar g\u00fc\u00e7l\u00fc bir a\u011fr\u0131 kesici yan\u0131t olu\u015fturur ve yan etkisi yoktur. Eski bilgilerimizde bu al\u0131c\u0131lar\u0131 tetikleyen herhangi bir ilac\u0131n a\u015fa\u011f\u0131 yukar\u0131 ayn\u0131 i\u015fi yapt\u0131\u011f\u0131n\u0131 d\u00fc\u015f\u00fcn\u00fcrd\u00fck. Oysa ger\u00e7ekte, farkl\u0131 \u015fekillerdeki ila\u00e7 molek\u00fclleri ayn\u0131 al\u0131c\u0131lar\u0131 farkl\u0131 yollarla tetikleyip farkl\u0131 yan\u0131tlara yol a\u00e7abilmektedir. Yani ortak bir \u00e7al\u0131\u015fmayla daha g\u00fc\u00e7l\u00fc bir etki yarat\u0131labilecektir. \u00b5 al\u0131c\u0131lar\u0131 olgusunda endomorfin-1 \u00f6zel bir se\u00e7icilikle a\u011fr\u0131 kesici etkiyi desteklemi\u015f olarak izlenmi\u015ftir.<\/p>\n<p>Dola\u015f\u0131mdaki seviyeleri \u00e7ok h\u0131zl\u0131 azald\u0131\u011f\u0131ndan do\u011fal endomorfinler ila\u00e7 olarak umut vermiyor. Zadina ve arkada\u015flar\u0131 kimyasal yap\u0131y\u0131 aynen olu\u015fturacak endomorfin benze\u015flerini (analog) laboratuvarda test etmeye ba\u015flad\u0131lar. Umutlar\u0131, bu molek\u00fcllerin, esas materyalle benzer olacak bir yan\u0131t\u0131 tetiklemesiydi. Zadian bu yeterlilikte d\u00f6rt yeni benze\u015f molek\u00fcl elde etti ve \u015fimdi klinik deneylerde en iyi yan\u0131t\u0131 verebilecek olan\u0131 se\u00e7meye \u00e7al\u0131\u015f\u0131yor. Analog 4 ad\u0131 verilen i\u00e7erik, morfinin yapt\u0131\u011f\u0131ndan daha uzun s\u00fcren bir a\u011fr\u0131 kesici etki s\u00fcreci yarat\u0131yor. Ayr\u0131ca daha az tolerans geli\u015ftiriyor ve a\u011fr\u0131 tetikleyici sitokinlerin sal\u0131n\u0131m\u0131na yol a\u00e7acak bir tetikleyicilik de yapm\u0131yor gibi g\u00f6z\u00fck\u00fcyor.<\/p>\n<p>Ek olarak analog 4 ba\u011f\u0131ml\u0131l\u0131k olu\u015fturmuyor. Zadina\u2019ya g\u00f6re en ikna edici veri farelerle yap\u0131lan klinik deneylerden geliyor.
