Duysal bilgiler sadece d\u0131\u015f uyaranlardan gelmez, kaslar\u0131m\u0131z, i\u00e7 organlar\u0131m\u0131z gibi i\u00e7sel uyaranlardan da gelir. Bu duyular, beyinde parietal lobda bulunan somatik duysal kortekste toplan\u0131r. Burada \u201cduysal homonkulus\u201d da denen, duysal bir beden temsili (haritas\u0131) vard\u0131r. Bu bedenimizin i\u015flevsel bir haritas\u0131d\u0131r, yani bedenimizde hangi b\u00f6lgeyi daha ayr\u0131nt\u0131l\u0131 i\u015flemlerde ve s\u0131k\u00e7a kullan\u0131yorsak, o b\u00f6lgenin temsil alan\u0131 daha geni\u015ftir. \u00d6rne\u011fin el ve y\u00fcz bedenin di\u011fer uzuvlar\u0131na g\u00f6re daha geni\u015f bir alan\u0131 temsil etmektedir. <\/em><\/p>\n<p>Antonio Damasio v\u00fccudun zihin i\u00e7in temel bir referans olu\u015fturdu\u011fundan bahseder. Ramachandran ise v\u00fccudu beynin bir hayaleti olarak tan\u0131mlar. Bu tan\u0131mlamalar bize bedenin zihnin bir yans\u0131mas\u0131 oldu\u011funu d\u00fc\u015f\u00fcnd\u00fcr\u00fcr. Hatta Damasio zihnin organizman\u0131n t\u00fcm\u00fcnden olu\u015ftu\u011funu s\u00f6yler. Zihin \u00fczerine d\u00fc\u015f\u00fcnceler, varolan bilgimiz artt\u0131k\u00e7a de\u011fi\u015fim g\u00f6stermektedir. 17. y\u00fczy\u0131lda zihnin nerede oldu\u011fu belirsizken 19. y\u00fczy\u0131lda Descartes\u2019\u0131n da katk\u0131s\u0131yla beynin i\u00e7inde ya da yak\u0131n \u00e7evresinde oldu\u011fu g\u00f6r\u00fc\u015f\u00fc h\u00e2kimdir. Damasio ise zihnin v\u00fccut-beyin etkile\u015fiminin kendisi oldu\u011funu ifade eder. Zihni sinir devrelerinin etkinli\u011fi olarak tan\u0131mlar. N\u00f6rofizyolog Charles Sherringtone bedenin pozisyon bilgisini (propriyosepsiyon) 6. duyu olarak tan\u0131mlar. Beden imgemiz \u00fc\u00e7 sistemden gelen bilgilerin entegrasyonuyla ger\u00e7ekle\u015fir. Bu sistemler g\u00f6rme, dokunma ve vestib\u00fcler (denge) sistemdir. Birindeki bir eksiklik di\u011fer sistem taraf\u0131ndan telafi edilmeye \u00e7al\u0131\u015f\u0131l\u0131r. G\u00f6zlerimizi kapad\u0131\u011f\u0131m\u0131zda, art\u0131k bedenimize ait pozisyon ve duyumlar\u0131 dokunma ve vestib\u00fcler sistem arac\u0131l\u0131\u011f\u0131yla alg\u0131lar\u0131z. Ya da vestib\u00fcler sistemin eksikli\u011finde g\u00f6rme duyusu bedenin pozisyon almas\u0131nda yard\u0131mc\u0131 olacakt\u0131r.<\/p>\n<p>Dokunma, d\u0131\u015f d\u00fcnyay\u0131 alg\u0131lamam\u0131z\u0131 sa\u011flayan temel be\u015f duyudan bir tanesidir. Bu elimizin ya da bedenimizin herhangi bir par\u00e7as\u0131n\u0131n bir y\u00fczeye dokunmas\u0131 (aktif dokunma) veya birinin ya da bir \u015feyin bedenimize temas\u0131 (pasif dokunma) \u015feklinde olabilir. Dokunma duyusu sayesinde bedenimize temas eden uyaran\u0131n so\u011fuklu\u011funu-s\u0131cakl\u0131\u011f\u0131n\u0131, sertli\u011fini-yumu\u015fakl\u0131\u011f\u0131n\u0131, ac\u0131 verici ya da ac\u0131s\u0131z bir uyaran m\u0131 oldu\u011funu biliriz. B\u00fct\u00fcn bu bilgileri derimizde bulunan \u00f6zel h\u00fccreler (resept\u00f6rler<sup>(1)<\/sup>) sayesinde ay\u0131rt ederiz. Bunlar y\u00fczeyel duyulard\u0131r (ekstroseptif duyular) ve t\u00fcm\u00fc de bizi d\u0131\u015f d\u00fcnya hakk\u0131nda bilgilendirir.<\/p>\n<p>Bedenimize ait bilgileri sadece d\u0131\u015f d\u00fcnyadaki uyaranlar olu\u015fturmaz. Bedenimizin kas\u0131lma ve gev\u015feme gibi hareketlerini kaslar ve eklemlerden gelen duyular sayesinde fark ederiz. Kas i\u00e7erisinde bulunan resept\u00f6rlerden gelen bu duyular\u0131 bilin\u00e7li derin duyu (propriyoseptif duyular), i\u00e7 organlar\u0131m\u0131za ait duyular\u0131 ise bilin\u00e7d\u0131\u015f\u0131 derin duyu (introseptif duyu) olarak tan\u0131mlar\u0131z.<sup>(2)<\/sup> Y\u00fczeyel ve derin duyular\u0131n olu\u015fturdu\u011fu bu sistem somatik duysal sistemdir. Somatik duyular bedendeki duysal bilgileri toplayan sinirsel mekanizmalard\u0131r. Beden i\u015flevlerine ait t\u00fcm duyular\u0131 somatik duyular alt\u0131nda inceleriz ve kendi i\u00e7erisinde b\u00f6l\u00fcmlere ayr\u0131l\u0131r.<\/p>\n<p>Bedenden gelen duyumlar\u0131n evrimsel a\u00e7\u0131dan bir i\u015flevselli\u011fi vard\u0131r. Bedeni korumak i\u00e7in gerekli davran\u0131\u015flar\u0131 \u00fcretebilmek beynin bedenin durumundan haberdar olmas\u0131yla m\u00fcmk\u00fcn olur. Dolay\u0131s\u0131yla bedene ili\u015fkin durumlar de\u011fi\u015fti\u011finde, beyin her seferinde bundan haberdar olmaktad\u0131r. Her haberdar edili\u015f, beyindeki beden temsilini her seferinde yenilemektedir.<\/p>\n<p><strong>Uyaranlar\u0131 kar\u015f\u0131layan organ: Deri ve al\u0131c\u0131lar\u0131<\/strong><\/p>\n<p>D\u0131\u015far\u0131dan gelen uyaranlar\u0131 alan organ\u0131m\u0131z deridir. Nesnelerin bi\u00e7imi, dokusu, y\u00fczeyi ve \u0131s\u0131s\u0131 deri \u00fczerindeki al\u0131c\u0131lar (resept\u00f6rler) sayesinde iletilmektedir. Ayn\u0131 zamanda deri v\u00fccudun s\u0131n\u0131rlar\u0131n\u0131 belirleyen bir ara\u00e7t\u0131r. Derinin derin y\u00fczeylerinde de iki mekanoresept\u00f6r vard\u0131r, bunlar bas\u0131n\u00e7la ilgili bilgiyi kodlarlar.<\/figcaption><\/figure>\n<p>Gelen uyaran hangi duysal sisteme aitse, o sisteme ait resept\u00f6rleri, sinir \u00a0yollar\u0131n\u0131 ve beyin merkezlerini harekete ge\u00e7irmektedir. Bu resept\u00f6rler duysal bilgiyi sinir sinyallerine (aksiyon potansiyeli<sup>(3)<\/sup>) \u00e7evirir ve merkezi sinir sistemine iletir. Bilindi\u011fi \u00fczere merkezi sinir sistemi beyin ve omurilikten olu\u015fur. \u00c7evreden ve i\u00e7 organlardan gelen duyular\u0131 ta\u015f\u0131yan bu sinirler omurilikte toplan\u0131r ve duyuyu beyne iletir. Peki gelen uyaran\u0131n dokusunu, \u0131s\u0131s\u0131n\u0131, rahats\u0131z edici olup olmad\u0131\u011f\u0131n\u0131, bedenimizin hangi noktas\u0131na temas etti\u011fini, \u015fiddetini nas\u0131l ay\u0131rt etmekteyiz?<\/p>\n<p>Somatik duyular\u0131n ay\u0131rt edilmesi farkl\u0131 resept\u00f6rlerin uyar\u0131lmas\u0131yla ger\u00e7ekle\u015fir. Her bir resept\u00f6r farkl\u0131 \u00f6zellikte duyular\u0131 al\u0131r. Bedenimizin baz\u0131 b\u00f6lgelerinde di\u011ferlerine g\u00f6re daha y\u00fcksek d\u00fczeyde hassasiyet s\u00f6z konusudur. Dokunulan b\u00f6lgedeki resept\u00f6r say\u0131s\u0131 bu hassasiyeti belirler. \u00d6rne\u011fin el \u00fczerindeki deride bulunan resept\u00f6r say\u0131s\u0131 bedenin di\u011fer b\u00f6lgelerine g\u00f6re daha fazlad\u0131r. Bu sayede elimiz nesnelerin karma\u015f\u0131k \u00f6zelliklerini ay\u0131rt etmekten Braille alfabesini<sup>(4) <\/sup>(k\u00f6rler alfabesi) okumaya kadar \u00e7ok say\u0131da i\u015flevi yerine getirmemizi sa\u011flar. Resept\u00f6r say\u0131s\u0131n\u0131n yan\u0131 s\u0131ra resept\u00f6rlerin adaptif \u00f6zellikleri de, uyaran\u0131n h\u0131z\u0131n\u0131 veya \u015fiddetini ay\u0131rt etmede yard\u0131mc\u0131 olur. E\u011fer bir uyaran dakikalar boyunca genli\u011fi ya da pozisyonu de\u011fi\u015fmeden devam ederse, n\u00f6ral tepki azal\u0131r ve duyu kaybolur, buna <strong>resept\u00f6r adaptasyonu <\/strong>denir. Bu sayede kolumuzdaki saati, parma\u011f\u0131m\u0131zdaki y\u00fcz\u00fc\u011f\u00fc ya da \u00fczerimizdeki k\u0131yafetleri her an hissetmeyiz. Denize girerken ilk anda so\u011fukluk hissetmemize ra\u011fmen k\u0131sa bir s\u00fcre sonra bu his kaybolur. Ba\u015flang\u0131\u00e7taki uyar\u0131 e\u015fik de\u011feri a\u015f\u0131p aksiyon potansiyelini ba\u015flatmaya yeterli oldu\u011fu halde, uyaran ayn\u0131 d\u00fczeyde gelmeyi s\u00fcrd\u00fcrd\u00fck\u00e7e duyu al\u0131c\u0131lar\u0131n\u0131n (resept\u00f6rlerin) yeni aksiyon potansiyeli ba\u015flatma g\u00fcc\u00fc zay\u0131flar.<\/p>\n<p>Dokunma, \u0131s\u0131, a\u011fr\u0131, kas ve eklemlere ait duyular s\u0131ras\u0131yla kutan\u00f6z mekanoresept\u00f6rler, termal resept\u00f6rler, nosisept\u00f6rler ve kas-ekleme ait mekanoresept\u00f6rlerle<sup>(5)<\/sup> ta\u015f\u0131n\u0131rlar. Resept\u00f6rlerden \u00e7\u0131kan sinyaller merkezi sinir sistemine (MSS\u2019ye) farkl\u0131 sinir lifleriyle ta\u015f\u0131n\u0131rlar. Bu sinir lifleri, etraflar\u0131ndaki koruyucu, ayn\u0131 zamanda iletken k\u0131l\u0131f\u0131n (miyelin k\u0131l\u0131f\u0131) \u00e7ap\u0131na g\u00f6re farkl\u0131la\u015fmaktad\u0131r. Sinir lifinin \u00e7ap\u0131 b\u00fcy\u00fcd\u00fck\u00e7e MSS\u2019ye iletim h\u0131z\u0131 da artar. Dokunmay\u0131 ve derin duyular\u0131 (propriyoseptif) kal\u0131n \u00e7apl\u0131 lifler, \u0131s\u0131 ve a\u011fr\u0131 duyusunu ise ince \u00e7apl\u0131 lifler ta\u015f\u0131r. Dolay\u0131s\u0131yla bu duyular\u0131n iletim h\u0131zlar\u0131 farkl\u0131la\u015fmaktad\u0131r. Sinir liflerinin etraf\u0131ndaki koruyucu yap\u0131n\u0131n (miyelin) hasarlanmas\u0131 ise duyu kay\u0131plar\u0131na neden olur. \u00d6rne\u011fin diyabette n\u00f6ropatinin g\u00f6r\u00fclmesi ya da multiple sklerozda sinir iletim h\u0131z\u0131n\u0131n yava\u015flamas\u0131, duyular\u0131n iletiminde azalma ya da kay\u0131plarla sonu\u00e7lan\u0131r.<\/p>\n<p>Dokunma duyusunda 4 temel mekanoresept\u00f6rden bahsedebiliriz. Bunlardan ikisi derinin y\u00fczeyinde bulunurken (Meissner ve Merkel) di\u011fer ikisi derinin daha derin tabakalar\u0131na (Pasini ve Ruffini) yerle\u015fmi\u015ftir. Bu resept\u00f6rlerin her biri deriye ait farkl\u0131 bir bilgiyi ta\u015f\u0131r. \u00d6rne\u011fin Meisner ve Merkel daha \u00e7ok parmak ucunda bulunur ve Braille alfabesini okurken ate\u015flenir. Pasini ve Ruffini ise bas\u0131n\u00e7la ilgili bilgiyi kodlar.<\/p>\n<p>A\u011fr\u0131 dokunma duyusundan farkl\u0131 resept\u00f6rlerce alg\u0131lan\u0131r, bunlara nosisept\u00f6r denir. \u0130\u011fnenin deriye batmas\u0131 ya da derinin bir yere s\u0131k\u0131\u015fmas\u0131 s\u0131ras\u0131nda nosisept\u00f6rler devreye girer. A\u015f\u0131r\u0131 s\u0131cak ya da so\u011fukla temasta da ayn\u0131 nosisept\u00f6rler uyar\u0131l\u0131r. Derideki resept\u00f6rlerden \u00e7\u0131kan bilgi (sinyal) g\u00f6t\u00fcr\u00fcc\u00fc (affarent) liflerle omurili\u011fe ta\u015f\u0131n\u0131r. Dokunma ve pozisyon (propriyosepsiyon) bilgisi ile a\u011fr\u0131 ve \u0131s\u0131 bilgisi beyine 2 farkl\u0131 yoldan ta\u015f\u0131n\u0131r ve omurili\u011fin farkl\u0131 d\u00fczeylerinde \u00e7aprazla\u015f\u0131rlar. Bilindi\u011fi \u00fczere beyinde \u00e7apraz i\u015flemleme vard\u0131r. Yani beynin sa\u011f hemisferi bedenin sol taraf\u0131ndan gelen bilgileri al\u0131p i\u015flerken, beynin sol hemisferi bedenin sa\u011f taraf\u0131ndan gelen bilgileri kodlar, yorumlar ve y\u00f6netir. Bu durumda sa\u011f elimize bir i\u011fne batt\u0131\u011f\u0131nda bunun bilgisi sol hemisfere iletilecektir. Resept\u00f6rler taraf\u0131ndan omurili\u011fe ta\u015f\u0131nan sinyaller beyinde koku hari\u00e7 t\u00fcm duyular\u0131n ara istasyonu olan talamusa u\u011frayarak duysal kortekse girerler ve b\u00f6ylece biz elimize batan i\u011fneyi alg\u0131lar\u0131z.<\/p>\n<p>Anla\u015f\u0131ld\u0131\u011f\u0131 \u00fczere, duysal bilginin beyne ta\u015f\u0131nmas\u0131 \u00e7e\u015fitli a\u015famalardan sonra ger\u00e7ekle\u015fmesine ra\u011fmen, iletim olduk\u00e7a h\u0131zl\u0131d\u0131r. Duysal bilginin ta\u015f\u0131n\u0131rken ge\u00e7ti\u011fi yollardan herhangi birinde olu\u015facak bir hasar, duysal kay\u0131plara neden olabilmektedir. Oliver Sacks, <em>Kar\u0131s\u0131n\u0131 \u015eapka Sanan Adam<\/em> kitab\u0131nda, \u00e7evresel sinirlerin iltihab\u0131 (polinevrit) sonucu, bedenle ilgili duyumunu kaybeden bir hastadan bahseder. Propriyoseptif bilgiyi ta\u015f\u0131yan liflerin hasar\u0131 hastan\u0131n bedeninin pozisyonuna ili\u015fkin bilgiden mahrum kalmas\u0131na ve ayaklar\u0131n\u0131n \u00fczerinde duramamas\u0131na, eline ald\u0131\u011f\u0131 \u015feyleri hissedemedi\u011fi i\u00e7in d\u00fc\u015f\u00fcrmesine neden olur. Kitapta hastan\u0131n ya\u015fad\u0131\u011f\u0131 durum beden k\u00f6rl\u00fc\u011f\u00fc olarak ifade edilir. Bu bozuklukta hastalar, \u00f6rne\u011fin ceket giyerken fel\u00e7li beden yar\u0131s\u0131n\u0131 ihmal ederler, etkilenen beden yar\u0131s\u0131n\u0131 y\u0131kamaz ya da tra\u015f etmezler. Yatakta yatarken baca\u011f\u0131n\u0131n kendisine ait oldu\u011funu ink\u00e2r eder, hatta \u201cKim koydu bu baca\u011f\u0131 yata\u011f\u0131ma\u201d diyebilirler. Anozognozinin ilgin\u00e7 bir y\u00f6n\u00fc kendini hep sol beden yar\u0131s\u0131n\u0131n ihmaliyle g\u00f6stermesi, yani sa\u011f hemisferin hasar\u0131nda g\u00f6r\u00fclmesidir.
