Klinik \u00e7al\u0131\u015fmada <strong>transtiretin amiloidoz<\/strong> ad\u0131 verilen, nadir ve \u00f6l\u00fcmc\u00fcl hastal\u0131\u011fa sahip alt\u0131 ki\u015fi, gen d\u00fczenleme mekanizmas\u0131 kullan\u0131larak tedavi edilmeye \u00e7al\u0131\u015f\u0131ld\u0131. Hastalar\u0131n t\u00fcm\u00fcnde hastal\u0131kla ili\u015fkili bir proteinin seviyesinde d\u00fc\u015f\u00fc\u015f ya\u015fand\u0131. Tedavinin dozu art\u0131r\u0131ld\u0131\u011f\u0131nda TTR ad\u0131 verilen proteinin seviyesinde y\u00fczde 87\u2019lik bir d\u00fc\u015f\u00fc\u015f ya\u015fand\u0131\u011f\u0131 g\u00f6r\u00fcld\u00fc.<\/p>\n<p>CRISPR\u2013Cas9 ile ili\u015fkili deneylerden elde edilen \u00f6nceki sonu\u00e7lar, tekni\u011fin v\u00fccuttan \u00e7\u0131kar\u0131lm\u0131\u015f h\u00fccreler \u00fczerinde kullan\u0131labilece\u011fine i\u015faret etmi\u015fti. Bu durumda kat\u0131l\u0131mc\u0131lar\u0131n v\u00fccudundan \u00e7\u0131kar\u0131lm\u0131\u015f h\u00fccreler d\u00fczenlenip kat\u0131l\u0131mc\u0131lara yeniden veriliyordu. Fakat genleri v\u00fccut i\u00e7erisinde d\u00fczenleyebilmek, daha geni\u015f bir hastal\u0131k yelpazesine kar\u015f\u0131 tedavi geli\u015ftirilmesini sa\u011flayacakt\u0131r.<\/p>\n<p>MIT\u2019de biyomedikal m\u00fchendisi olarak \u00e7al\u0131\u015fmalar\u0131n\u0131 s\u00fcrd\u00fcren olan Daniel Anderson, \u201cBu alan i\u00e7in \u00f6nemli bir an, t\u0131pta yeni bir d\u00f6nem\u201d diyerek \u00e7al\u0131\u015fman\u0131n \u00f6nemini vurgulamaktad\u0131r.<\/p>\n<p><strong>Tedavi edilemeyenleri tedavi etmek<\/strong><\/p>\n<p>Transtiretin amiloidoz hastal\u0131\u011f\u0131, TTR proteinlerinin yanl\u0131\u015f bir \u015fekilde katlan\u0131p birikerek organlar\u0131n \u00e7al\u0131\u015fma mekanizmas\u0131na m\u00fcdahale etmesiyle ortaya \u00e7\u0131kar. Hastal\u0131k esas olarak kalbe ve sinirlere sald\u0131r\u0131r. Hastal\u0131k kal\u0131tsal olabildi\u011fi gibi mutasyonlarla da ortaya \u00e7\u0131kabilir. Hastal\u0131\u011f\u0131n kal\u0131tsal versiyonu d\u00fcnya \u00fczerinde yakla\u015f\u0131k 50.000 ki\u015fiyi etkilemektedir. Buna ek olarak TTR geninde meydana gelen 100\u2019\u00fcn \u00fczerindeki mutasyon transtiretin amiloidoz hastal\u0131\u011f\u0131 ile ba\u011flant\u0131l\u0131d\u0131r. Bu hastal\u0131k kalp rahats\u0131zl\u0131klar\u0131na, a\u011fr\u0131ya ve organ i\u015flevlerini etkilemesi nedeniyle \u00f6l\u00fcme sebep olabilir. Yakla\u015f\u0131k 25 y\u0131ld\u0131r farkl\u0131 formlardaki amiloidoz hastal\u0131klar\u0131na sahip olan hastalar\u0131n\u0131 tedavi etmek i\u00e7in \u00e7al\u0131\u015fan nefrolog Julian Gillmore, \u201cBu kesinlikle korkun\u00e7 bir hastal\u0131k. Birka\u00e7 y\u0131l \u00f6ncesine kadar hastalar daha da k\u00f6t\u00fcle\u015fip hayat\u0131n\u0131 kaybediyordu\u201d a\u00e7\u0131klamalar\u0131nda bulundu.<\/p>\n<p>Hastal\u0131\u011f\u0131n y\u0131k\u0131c\u0131l\u0131\u011f\u0131, bilim d\u00fcnyas\u0131n\u0131 yeni tedavi metotlar\u0131 geli\u015ftirmeye y\u00f6neltti. Transtiretin amiloidoz tedavisi i\u00e7in hastal\u0131\u011f\u0131na neden olan genleri devre d\u0131\u015f\u0131 b\u0131rakmay\u0131 ama\u00e7layan yeni teknolojiler ana \u00e7al\u0131\u015fma konusu haline geldi. Transtiretin amiloidoz belirli bir proteinin \u00fcretimiyle ba\u011flant\u0131l\u0131d\u0131r. E\u011fer o proteinin \u00fcretimi durdurulursa, semptomlar da ilerlemeyi durdurur. Fakat baz\u0131 durumlarda mekanizma i\u015flemeye devam edebilir.<\/p>\n<p>Amerikan G\u0131da ve \u0130la\u00e7 Dairesi, transtiretin amiloidozu tedavisinde kullan\u0131lmak \u00fczere geli\u015ftirilen ve her ikisi de s\u00f6z konusu proteini kodlayan haberci RNA molek\u00fcllerini hedef alarak TTR \u00fcretimi azaltan iki ilaca 2018 y\u0131l\u0131nda kullan\u0131m onay\u0131 verdi. Fakat bu ila\u00e7lar TTR molek\u00fcllerini yakla\u015f\u0131k y\u00fczde 80 oran\u0131nda azalt\u0131p sa\u011fkal\u0131m s\u00fcresini uzatsa da, TTR d\u00fczeylerinin d\u00fc\u015f\u00fck bir seviyede tutulabilmesi i\u00e7in hastalar\u0131n ilac\u0131 rutin olarak kullanmas\u0131 gerekir. Etkinlikleri g\u00f6sterilmi\u015f olsa da ila\u00e7lar her zaman hastal\u0131\u011f\u0131n ilerlemesi durdurmayabilir. Bu durumda v\u00fccut i\u00e7erisindeki pek \u00e7ok mekanizma etkili olabilir.<\/p>\n<p>TTR proteinlerinin \u00e7o\u011fu, ila\u00e7lar\u0131 kandan emen, kan\u0131 bu anlam\u0131yla temizleyen karaci\u011ferde \u00fcretilir. Bu \u00f6zellik hastal\u0131\u011fa kar\u015f\u0131 CRISPR kullan\u0131m\u0131 i\u00e7in bir anahtar konumundad\u0131r. CRISPR\u2013Cas9, hastal\u0131klar i\u00e7in yeni ve son derece etkin tedavi metotlar\u0131 geli\u015ftirmenin bir yolu olsa da belirli s\u0131n\u0131rl\u0131l\u0131klar\u0131 vard\u0131r. Tekni\u011fi uygulayabilmek i\u00e7in Cas9 ad\u0131 verilen ve DNA\u2019y\u0131 kesen bir enzim ve onu DNA\u2019da hedeflenen b\u00f6lgeye y\u00f6nlendirecek bir k\u0131lavuz RNA\u2019ya ihtiya\u00e7 vard\u0131r. T\u00fcm bu yap\u0131lar bozulmadan korunacak bi\u00e7imde paketlenmeli ve v\u00fccutta hedeflenen b\u00f6lgeye g\u00f6nderilmelidir.<\/p>\n<p><strong>TTR \u00fcretiminde y\u00fczde 96\u2019ya varan azalma<\/strong><\/p>\n<p>\u00c7al\u0131\u015fmada, k\u0131lavuz RNA ve Cas9 proteini karaci\u011fer taraf\u0131ndan al\u0131nabilen lipit yap\u0131daki molek\u00fcller ile kapland\u0131. Mekanizma \u015f\u00f6yle i\u015flemekteydi: Hedeflenen b\u00f6lgeye bozulmadan ula\u015fan k\u0131lavuz RNA, TTR genini kesecek olan Cas9\u2019u hedef gene g\u00f6nderir. H\u00fccrenin DNA onar\u0131m s\u00fcre\u00e7leri bir s\u00fcre sonra DNA\u2019daki k\u0131r\u0131lmay\u0131 onar\u0131r fakat bu onar\u0131m s\u0131ras\u0131nda \u00e7al\u0131\u015fmada hedeflenen geni (TTR geni) devre d\u0131\u015f\u0131 b\u0131rakabilecek ve TTR proteininin \u00fcretimini durdurabilecek hatalar meydana gelebilir. B\u00f6ylece hastal\u0131\u011f\u0131n temel nedeni ortadan kald\u0131r\u0131l\u0131r veya etkisi \u00e7ok y\u00fcksek bir oranda azalt\u0131labilir.<\/p>\n<p>\u00c7al\u0131\u015fmada CRISPR-Cas9 kullan\u0131larak tedavi edilen kat\u0131l\u0131mc\u0131larda TTR proteini \u00fcretimi azald\u0131. Tedaviyi y\u00fcksek dozda alan bir kat\u0131l\u0131mc\u0131da ise TTR proteini d\u00fczeyi y\u00fczde 96 oran\u0131nda azald\u0131.<\/p>\n<p>Ara\u015ft\u0131rmac\u0131lar, daha fazla kat\u0131l\u0131mc\u0131yla \u00e7al\u0131\u015fmalar\u0131n\u0131 geni\u015fleteceklerini ve CRISPR-Cas9 kullan\u0131larak tedavi edilen kat\u0131l\u0131mc\u0131lar\u0131n sa\u011fl\u0131k durumlar\u0131n\u0131n uzun bir s\u00fcre boyunca takip edilece\u011fini s\u00f6yl\u00fcyor. B\u00f6ylelikle CRISPR-Cas9\u2019un hastal\u0131klara kar\u015f\u0131 do\u011frudan kullan\u0131m\u0131 i\u00e7in teknolojinin g\u00fcvenli\u011fi kapsaml\u0131 bir bi\u00e7imde izlenebilir. Yine de bu \u00e7al\u0131\u015fma hastal\u0131klara kar\u015f\u0131 CRISPR teknolojileri kullan\u0131m\u0131 konusunda \u00f6nemli veriler sunmaktad\u0131r. Geli\u015ftiricilerine 2020 Nobel Fizyoloji veya T\u0131p \u00d6d\u00fcl\u00fc verildi. CRISPR teknolojilerinin yeni mekanizmalar\u0131n ke\u015ffi, hastal\u0131klara kar\u015f\u0131 daha etkin tedavi metotlar\u0131n\u0131n geli\u015ftirilmesi gibi pek \u00e7ok s\u00fcrece \u00f6nemli katk\u0131lar sa\u011flayabilece\u011fi d\u00fc\u015f\u00fcn\u00fcl\u00fcyor. Bu konudaki \u00e7al\u0131\u015fmalardan elde edilen bulgular da sav\u0131 g\u00fc\u00e7lendirmektedir. Fakat CRISPR kullan\u0131m\u0131n\u0131n etik boyutlar\u0131 da tart\u0131\u015fmalara konu oluyor. Bir yan\u0131 ile CRISPR\u2019\u0131n tart\u0131\u015fmal\u0131 bir teknoloji oldu\u011fu d\u00fc\u015f\u00fcn\u00fclse de, teknolojinin g\u00fcvenlikli kullan\u0131m\u0131yla belki de CRISPR\u2019dan haberimizin olmad\u0131\u011f\u0131 bir d\u00fcnyada \u00fcstesinden gelemeyece\u011fimiz hastal\u0131klar\u0131n tedavisi i\u00e7in CRISPR\u2019l\u0131 d\u00fcnyada son derece etkin \u00e7\u00f6z\u00fcmler \u00fcretebiliriz.
