Temel olarak bir ya\u015flanma hastal\u0131\u011f\u0131 olmas\u0131na kar\u015f\u0131n k\u00f6keni \u00e7ocukluktaki geli\u015fimsel d\u00f6nemlere dayanan birikimli bir hastal\u0131kt\u0131r.<br \/>\nKemik dokunun temel g\u00f6revi bedeni hareket ettiren mekanik g\u00f6revler olmas\u0131na kar\u015f\u0131n lokomotor i\u015flevin \u00f6n\u00fcne ge\u00e7en \u00e7ok daha \u00f6nemli rolleri bulunmaktad\u0131r. Organizmalar i\u00e7in hem mineral deposudur hem de mineral homeostaz\u0131nda etkin rol oynar. Bu \u00f6zelli\u011fi hem soy a\u011fac\u0131nda hem de geli\u015fimsel s\u00fcre\u00e7lerde ya\u015fam olaylar\u0131n\u0131 y\u00f6nlendiren dinamik bir s\u00fcre\u00e7tir. V\u00fccuttaki total kalsiyum ve fosforun % 97\u2019si kemik dokuda depolan\u0131r. Ek olarak, metabolik ve \u00fcreme i\u015flevlerine sahip endokrin organd\u0131r.<br \/>\nOsteoporoz modern \u00e7a\u011f\u0131n yayg\u0131n bir hastal\u0131\u011f\u0131d\u0131r. Ya\u015fl\u0131l\u0131k sorunlar\u0131n\u0131n \u00f6nde gelen nedenlerindendir. Halen y\u00fcz milyonlarca insan osteoporozdan etkilenmi\u015f bulunmaktad\u0131r ve \u00f6n\u00fcm\u00fczdeki y\u0131llarda erkeklerde 2,5, kad\u0131nlarda 3 kat art\u0131\u015f h\u0131z\u0131 ile epidemik bir d\u00fczeye ula\u015f\u0131laca\u011f\u0131 \u00f6ng\u00f6r\u00fclmektedir.<\/p>\n<p><strong>Osteoporozu etkileyen fakt\u00f6rler<br \/>\n<\/strong>Osteoporozun temel belirleyeni eri\u015fkin kemik k\u00fctlesinin d\u00fczeyidir. Eri\u015fkin kemik birikiminin \u00e7o\u011funlu\u011fu \u00e7ocukluk ve erken eri\u015fkinlik d\u00f6neminde kazan\u0131l\u0131r. Birikimin en \u00f6nemli b\u00f6l\u00fcm\u00fc b\u00fcy\u00fcmenin devam etti\u011fi s\u00fcre\u00e7 boyunca kazan\u0131lmakla birlikte % 10\u2019a yak\u0131n b\u00f6l\u00fcm\u00fc b\u00fcy\u00fcmenin tamamlanmas\u0131ndan sonra da devam eder. O y\u00fczden osteoporozu \u00f6nleme giri\u015fimleri i\u00e7in en rasyonel yakla\u015f\u0131m bu d\u00f6neme odaklanmal\u0131d\u0131r. Doruk kemik kitlesi denilen bu nihai birikimin d\u00fczeyini genetik, fiziksel aktivite, ya\u015f, cinsiyet, hormonlar, cinsel olgunla\u015fma, beslenme gibi fakt\u00f6rler belirler. Bunlardan en \u00f6nemlisi fiziksel aktivite ile ili\u015fkili ya\u015fam tarz\u0131d\u0131r. Kemikler, \u00fczerine binen mekanik y\u00fcke adapte olacak bi\u00e7imde uyarlan\u0131r. Wolf\u2019un bu kuram\u0131 temelinde Frost\u2019un geli\u015ftirdi\u011fi <strong>mekanostat kuram\u0131na<\/strong> g\u00f6re kemikler, \u00fczerlerine eklenen yeni gerilim y\u00fcklerine kemik yap\u0131m\u0131n\u0131 uyararak yan\u0131t verirler. Ya\u015fa ve ya\u015fam tarz\u0131na uygun egzersiz ile osteoporoza ba\u011fl\u0131 k\u0131r\u0131klar\u0131n \u00fc\u00e7te birinin engellenebilece\u011fi g\u00f6sterilmi\u015ftir. Bu olumlu etki t\u00fcm ya\u015fam boyunca devam eder.<br \/>\nOsteoporozun % 60-80\u2019i genetik ile ili\u015fkilendirilmesine kar\u015f\u0131n genetik \u00f6zellikler k\u00f6k nedenin sadece 1\/5\u2019ini a\u00e7\u0131klamaktad\u0131r. Beyazlara oranla, Afrikal\u0131 Amerikal\u0131larda osteoporoz riski ve k\u0131r\u0131k s\u0131kl\u0131\u011f\u0131 daha azd\u0131r.<br \/>\nPubertal geli\u015fim ve cinsel olgunla\u015fman\u0131n doruk kemik kitlesi \u00fczerine belirgin bir etkisi bulunmaktad\u0131r. Puberteye (ergenlik d\u00f6nemi) giri\u015f ya\u015f\u0131 puberte evresi ve cinsiyet hormonlar\u0131 kemik geli\u015fimini ve osteoporoz durumunu etkiler. \u00d6strojen osteoblastlar\u0131 uyar\u0131p osteoklastlar\u0131 inhibe ederken, kalsiyum emilimini art\u0131r\u0131rlar ve her iki cinsiyette epifizlerin (kemik b\u00fcy\u00fcme hatt\u0131) kapanmas\u0131n\u0131 etkilerler. \u00d6strojenin dinamik de\u011fi\u015fkenlik g\u00f6sterdi\u011fi hamilelik, laktasyon, (s\u00fct verme) menopoz s\u00fcre\u00e7leri ve hipo\u00f6strojenizm (\u00f6strojen \u00fcretiminin azl\u0131\u011f\u0131) ile giden hastal\u0131klar da osteoporoz geli\u015fimini etkiler. Kad\u0131nlarda, hormonal dinamik fizyolojik s\u00fcre\u00e7ler nedeniyle osteoporoz erkeklerden iki kat daha fazla g\u00f6r\u00fcl\u00fcr. B\u00fcy\u00fcme hormonu, IGF-1 (b\u00fcy\u00fcme hormonunun etkisini onun \u00fczerinden ger\u00e7ekle\u015ftirdi\u011fi protein) ve osteokalsin gibi kalsiyotropik (kalsiyum metabolizmas\u0131 lehine) hormonlar\u0131n da kemik sa\u011fl\u0131\u011f\u0131 \u00fczerine etkileri bulunmaktad\u0131r.<br \/>\n\u00d6zellikle diyette yeterli kalsiyum ve vitamin D al\u0131m\u0131n\u0131n yan\u0131nda A, C ve K vitaminleri kemik geli\u015fimini olumlu etkilerken gazl\u0131 i\u00e7ecekler, kafein, alkol ve proteinden a\u015f\u0131r\u0131 zengin diyet osteoporoz riskini art\u0131r\u0131r.<\/p>\n<p><strong>Osteoporoza evrimsel yakla\u015f\u0131m<br \/>\n<\/strong>Uygarl\u0131k hastal\u0131klar\u0131 olarak bilinen obezite, kalp damar hastal\u0131klar\u0131, otoimm\u00fcn hastal\u0131klar, diyabet, metabolik sendrom, bel f\u0131t\u0131klar\u0131 gibi osteoporoz da modern insan\u0131n yayg\u0131n g\u00f6r\u00fclen bir hastal\u0131\u011f\u0131d\u0131r. Di\u011fer uygarl\u0131k hastal\u0131klar\u0131nda oldu\u011fu gibi osteoporoza da evrimsel yakla\u015f\u0131m \u00f6nemlidir, \u00e7\u00fcnk\u00fc g\u00fcncel klinik durumlar ge\u00e7mi\u015fimizin evrimsel tasar\u0131m\u0131d\u0131r. Osteoporozun evrimine ge\u00e7i\u015f yapmadan \u00f6nce evrimsel t\u0131p yakla\u015f\u0131mlar\u0131n\u0131n \u00f6nem ve yarar\u0131na ili\u015fkin k\u0131sa bir giri\u015f yapmak gerekti\u011fine inan\u0131yorum.<br \/>\n<strong>Evrimsel t\u0131p<\/strong>, hastal\u0131klar\u0131n g\u00fcncel mekanizmalar\u0131n\u0131n \u00fczerine in\u015fa edildi\u011fi ve ge\u00e7mi\u015f atasal t\u00fcrlerden miras al\u0131nan potansiyel ve k\u0131s\u0131tl\u0131l\u0131klarca bi\u00e7imlenmi\u015f nihai k\u00f6k temeli esas alan bir bilim disiplinidir. Bu bak\u0131\u015f a\u00e7\u0131s\u0131na g\u00f6re hastal\u0131klar, \u00fcreme ve hayatta kalma program\u0131na katk\u0131 sa\u011flayan yararl\u0131 adaptasyonlara kar\u015f\u0131 \u00f6d\u00fcnle\u015fmede verilmesi gereken bedellerdir. Hastal\u0131klar da di\u011fer t\u00fcm organizmalara \u00f6zg\u00fc fenotipler gibi genler ve \u00e7evre etkile\u015fiminin do\u011fal se\u00e7ilimce belirlenen karma\u015f\u0131k bir spektrumu olarak bi\u00e7imlenirler. Evrimsel t\u0131bba g\u00f6re, hastal\u0131klar atasal t\u00fcrlerden miras al\u0131nan genotip yap\u0131 ile i\u00e7ine do\u011fulan \u00e7evre aras\u0131ndaki uyumsuzluklard\u0131r. T\u00fcr d\u00fczeyinde yayg\u0131n hale gelmi\u015f, t\u00fcre \u00f6zg\u00fc hastal\u0131klar g\u00fc\u00e7l\u00fc bir do\u011fal se\u00e7ilim bask\u0131s\u0131 alt\u0131ndaki ge\u00e7mi\u015f \u00e7evresel ko\u015fullar i\u00e7in uyarlanm\u0131\u015f \u00f6zelliklerdir. Dolay\u0131s\u0131yla g\u00fcncel hastal\u0131klar\u0131n k\u00f6k nedenini, onlar\u0131 yaratan ge\u00e7mi\u015fteki evrimsel g\u00fc\u00e7leri bilmek g\u00fcncel \u00e7evrede hastal\u0131klar olarak yans\u0131yan \u00f6zelliklerin etyoloji, yayg\u0131nl\u0131k ve patogenezini kavramada yararl\u0131 bir anahtar, de\u011ferli bir mercek sunar. Gelecekteki potansiyel tedavi yakla\u015f\u0131mlar\u0131 i\u00e7in rasyonel bir \u00f6ng\u00f6r\u00fc sa\u011flar. Kuram, do\u011fal se\u00e7ilimin temel ya\u015fam becerilerine (b\u00fcy\u00fcme, \u00fcreme, onar\u0131m, yavru bak\u0131m\u0131) enerji tahsisat\u0131n\u0131n\/uzla\u015f\u0131n\u0131n filogenetik s\u0131n\u0131rl\u0131l\u0131klar temelinde geli\u015fimsel s\u00fcre\u00e7lere \u00f6zg\u00fc zamanlamay\u0131 nas\u0131l \u00fcretti\u011fine y\u00f6nelik kapsaml\u0131 a\u00e7\u0131klamalar sunar. Ya\u015fa ve d\u00f6neme \u00f6zg\u00fc b\u00fcy\u00fcme, \u00fcreme ve ya\u015flanma kal\u0131plar\u0131na evrimsel a\u00e7\u0131klamalar yapar. Bu ba\u011flamda osteoporoz insan evriminin bir hastal\u0131\u011f\u0131d\u0131r. Bu yakla\u015f\u0131m, filogenetik s\u00fcre\u00e7ler boyunca kemik b\u00fct\u00fcnl\u00fc\u011f\u00fcn\u00fc etkileyen evrimsel g\u00fc\u00e7lerle g\u00fcncel geli\u015fimsel s\u00fcre\u00e7ler aras\u0131ndaki karma\u015f\u0131k etkile\u015fimi anlamaya y\u00f6nelik derinlikli bir \u00e7er\u00e7eve sunar. Evrimsel t\u0131p yakla\u015f\u0131m\u0131na g\u00f6re osteoporoz, ge\u00e7mi\u015fteki yararl\u0131 evrimsel adaptasyonlar\u0131n g\u00fcn\u00fcm\u00fcz \u00e7evresel ko\u015fullar\u0131nda ortaya \u00e7\u0131kan bir yan \u00fcr\u00fcn\u00fcd\u00fcr. Bu yakla\u015f\u0131ma g\u00f6re osteoporoz, ge\u00e7mi\u015fte kazan\u0131lan bipedalizme (iki ayakl\u0131l\u0131k) ve uzam\u0131\u015f ya\u015fam s\u00fcresine kar\u015f\u0131 verilen bedellerdir. Koroner kalp hastal\u0131klar\u0131, Tip 2 diyabet, obezite, metabolik sendrom, bel f\u0131t\u0131klar\u0131 gibi pek \u00e7ok kronik hastal\u0131k da evrimsel temele dayanan genetik ve \u00e7evre aras\u0131ndaki uyum bozukluklar\u0131d\u0131r. Art\u0131k, neredeyse t\u00fcm insan hastal\u0131klar\u0131n\u0131n \u00e7evre-gen etkile\u015fiminin belirledi\u011fi dinamik bir spektrumda ortaya \u00e7\u0131kt\u0131\u011f\u0131, hatta tek gen hastal\u0131klar\u0131ndan sorumlu genlerin dahi \u00e7evresel de\u011fi\u015ftiricilerin etkisi alt\u0131nda ifade edildi\u011fine dair \u00e7ok say\u0131da kan\u0131t birikmi\u015ftir. T\u00fcm bunlar, osteoporozun k\u00fcresel bir yayg\u0131nl\u0131k kazanmas\u0131na neden oldu.<\/figcaption><\/figure>\n<p><strong>Kemik yap\u0131n\u0131n k\u00f6keni<br \/>\n<\/strong>Vertebral\u0131larda kemik dokunun k\u00f6keni ilkin ya\u015fam formlar\u0131ndaki biyomineralize sert d\u0131\u015f kabuk mimariye dayan\u0131r. Memelilere evrilen soy hatt\u0131nda kemik yap\u0131lar i\u00e7ine do\u011fduklar\u0131 \u00e7evreye g\u00f6re de\u011fi\u015fen \u00e7e\u015fitli roller \u00fcstlenmi\u015f ve uyum i\u00e7in iskelet fizyolojisine yeni hormonlar, peptidler eklemi\u015ftir. Kemik doku kollegenden olu\u015fan matriks, Ca, P temelinde mineraller ve osteosit, osteoklast ile osteoblastlar\u0131 i\u00e7eren canl\u0131 h\u00fccrelerden olu\u015fur. Bu 3 temel yap\u0131 filogenetik soy hatt\u0131nda peyderpey eklenmi\u015ftir.<br \/>\n\u0130skelet yap\u0131lar temel olarak iki farkl\u0131 k\u00f6kenden t\u00fcremi\u015ftir. \u00d6ncelikle, dermal yap\u0131lardan evrimle\u015fen ekzoskleton (d\u0131\u015f kemik) ortaya \u00e7\u0131km\u0131\u015ft\u0131r. Dermisten k\u00f6ken alan membran\u00f6z kemikle\u015fme \u00f6nce kraniumdan (kafatas\u0131) ba\u015flam\u0131\u015f, di\u015fler, pullar, y\u00fczge\u00e7 \u0131\u015f\u0131nlar\u0131 \u00fczerinden t\u00fcm v\u00fccut y\u00fczeyine yay\u0131lm\u0131\u015ft\u0131r. Dentin ve enema vertebral\u0131larda ilk kemikle\u015fme g\u00f6steren kemik yap\u0131lard\u0131r. K\u0131k\u0131rdaktan k\u00f6ken alan endoskleton (i\u00e7 kemik) vertebra, n\u00f6rokranium ve uzun ekstremiteleri olu\u015fturmu\u015ftur. Sucul ya\u015fam boyunca okyanuslardaki asiditede ve mineral i\u00e7eriklerinin de\u011fi\u015fimi, kemik fizyolojisinin evrimine yol a\u00e7m\u0131\u015ft\u0131r. Kambriyan D\u00f6nemi ile birlikte vertebral\u0131 ya\u015fam formlar\u0131 ortaya \u00e7\u0131km\u0131\u015ft\u0131r. Bu d\u00f6nemde artan avc\u0131lar, korunma ve daha h\u0131zl\u0131 hareket i\u00e7in bask\u0131 olu\u015fturmu\u015ftur. Kemik yap\u0131lar bir yandan sert yap\u0131lar olarak korunmaya hizmet ederken, di\u011fer yandan daha h\u0131zl\u0131 ve daha uzak mesafelere g\u00f6\u00e7 i\u00e7in gereken enerji gereksinimini sa\u011flayan batarya\/pil olarak hizmet eden mineral deposuna fiziksel ortam sa\u011flam\u0131\u015ft\u0131r.
